۱.من تأیید میکنم که ارائه هرگونه اطلاعات نادرست، ناقص یا گمراهکننده در این فرم، مسئولیت شخصی من است و ممکن است منجر به رد درخواست یا لغو صلاحیت من از دریافت خدمات و حمایتهای مربوطه شود.(I hereby confirm that submitting any false, incomplete, or misleading information in this form is solely my responsibility and may result in rejection of my application or disqualification from receiving any related services or benefits.)
۲.متعهد میشوم هیچ اقدامی، چه مستقل و چه مرتبط با این موضوع، بدون کسب رضایت قبلی شما انجام ندهم.(I undertake not to take any action, whether independently or in relation to this matter, without obtaining your prior consent.)
۳.میپذیرم در صورتی که هرگونه غرامت یا مطالبه مالی به نمایندگی از من وصول شود، پنجاه درصد (۵۰٪) از مبلغ بازیافتی به عنوان هزینه خدمات کسر گردد.(I agree that in the event any compensation or financial claim is successfully recovered on my behalf, a service fee of 50% of the total recovered amount shall be deducted as remuneration for services rendered.)
